Welcome to the 2006 Awards!

Before completing the Nomination Form below, please note the following:

• A link has been provided for you to review a Sample Nomination Form and required components before submitting your nomination.

• You are required to submit two items - the online nomination form and a multimedia component. The latter may be submitted as part of this nomination process or it may be mailed separately.

• You will receive a "Confirmation of Successful Completion of your Nomination" notice as soon as you press "Finish" at the end of the Nomination Form. If you do not receive this notice, your form has not been received. If problems persist, please contact us at techawards2006@thelearningpartnership.ca or (416) 481-0799.

• Nominated projects should focus on the innovative and creative use of technology and include areas such as blended learning environments, knowledge transfer, organizational learning, professional development and/or the needs of special education students.

• Please review the Selection Criteria and Judging Rubric that will be used in judging the 2006 nominations. It may also be helpful to review the 2005 winning projects.

• Award categories will include: Regional (Western and Northern Canada, Central Canada (Ontario and Quebec), Eastern Canada), Education Sectors (elementary, secondary, district/provincial, college/university) a Judges' Award and a Conference Award.

• Entries, including multimedia component, must be received by 5:00 p.m. EST, Friday, February 17, 2006
